1	Family Particulars: Damayanthi’s family consists of 5 members, herself (37), her husband (39) and two children (13 and 10) who attend school. They earn their living through farming and work on their family land. They did not suffer personal injury but lost some personal belongings they had. Children’s school materials were destroyed by flood and they did not attend school for 2 weeks as Damayanthi could not find any casual labour and did not have any money. They are not indebted. Need : Personal needs and children’s school needs must be provided.	Rs.3,000 US$.30.-
2	House: The house was made of clay walls and tin and cadjan roof. More than 60% of their house has been destroyed. It is possible to use some materials in building a house. They now live in a temporary shelter made of galvanised sheets. They need help to build a house. Need : Our partner NGO, Berendina Development Services (BDS) will build a house towards which we will make a contribution.	Rs.50,000 US$.500.-
3	Household Goods: They had an almirah (cupboard), bed, table and 6 chairs.. The almirah was fully damaged by the flood and some kitchen utensils were destroyed. Need : They need basic furniture and kitchen utensils.	Rs.4,000 US$,40.-
4	Vehicles: They don’t have any vehicles.	0
5	Farm Holding: They had cultivated ½ acre upland in chilli 3 months before the flood. They have spent around Rs. 12,000/- for their cultivation. This field was completely destroyed by flood. They have to restart cultivation in October.She wants to keep some goats. Need: They need support to cultivate their land for chilli.	chilli - 25,000 goat farming - 27,400 52,400 US$.524.-
6	Other: She earns about Rs.6,000/- from casual labour and gets Rs.615/- from Samurdhi. She takes things to sell at the pola (fair) and earns about Rs.2,000/-. Altogether about Rs.8,615/- a month Need : They need family support for 6 months @ Rs.2,500/- a month	Rs.15,000 US$.150.-
7	Total cost for Rehabilitation of Family	Rs.124,400 US$.1,244.-
8	Implementation Procedure: BDS has very kindly offered to undertake the field level implementation for the 20 families we are Adopting parallel to the projects it is implementing for other families in the area. Chakra Network Sri Lanka will take overall responsibility for the 20 families. This will involve receiving funds from Sponsoring Families / Individuals earmarked for the families they adopt, transferring funds to BDS for specific tasks in the Rehabilitation Plan for each Adopted Family, keeping track of funds spent for each Adopted Family and keeping the Sponsoring Families / Individuals informed of the progress in implementing the Rehabilitation Plan. This will incur some cost for Chakra Network Sri Lanka Need : Administration / Coordination cost	Rs.10,000 US$.100.-
