1	Family Particulars: Chandrasiri’s family consists of 04 members, himself (37), his wife (32) and two children (13 and 8) who attend school. They work on their family land to earn their living. They did not suffer personal injury but lost some their belongings. Children’s school materials were destroyed by flood. They have a loan of Rs.69,500/- which they will repay from their earnings from cultivation Personal needs and children’s school needs	Rs.4,000 US$.40.-
2	House: The house was made of clay walls, tin & kadjan roof and clay floor. Most of their house was destroyed/damaged by the flood. Now they live in a part of the house still standing. They need help to build a house. Need : Our partner NGO, Berendina Development Services (BDS) will build a house towards which we will make a contribution.	Rs.50,000 US$.500.-
3	Household Goods: They had little furniture and kitchen utensils. Radio cassette and kitchen utensils were destroyed by the flood.   Need : They need basic kitchen utensils and cassette radio.	Rs.3,500 US$.35.-
4	Vehicles: He had two push bicycles which were not damaged by the flood.	0
5	Farm Holding: He had cultivated 1 acre in brinjal. He has spent around Rs. 19,000/- for this cultivation. Also he had cultivated ½ acres in ribbed gourd (watakolu) and he has spent around Rs. 25,000/- for this cultivation.  And also he had cultivated 1 ½ acres in paddy and he has spent around Rs. 23,000/- for cultivate paddy. All crops planted 3 months before flood and all these cultivations were completely destroyed by flood and he lost totally around Rs. 67,000/- due to the flood. He has to restart cultivation in November. Need: They want replant their crops again and he would like to get a pesticides sprayer. 8	Implementation Procedure: BDS has very kindly offered to undertake the field level implementation for the 20 families we are Adopting parallel to the projects it is implementing for other families in the area. Chakra Network Sri Lanka will take overall responsibility for the 20 families. This will involve receiving funds from Sponsoring Families / Individuals earmarked for the families they adopt, transferring funds to BDS for specific tasks in the Rehabilitation Plan for each Adopted Family, keeping track of funds spent for each Adopted Family and keeping the Sponsoring Families / Individuals informed of the progress in implementing the Rehabilitation Plan. This will incur some cost for Chakra Network Sri Lanka Need : Administration / Coordination cost	5,000
