1	Family Particulars: Nilmini’s family consists of 04 members, herself (30) and two daughters (10 and 15) and her mother (60). Her husband died 3 years ago and she works on their family land to earn their living. Her elder dropped out of school when her husband died as she could buy her school requirements. The younger daughter attends school. They did not suffer personal injury but lost a lot of their belongings. Children’s school materials were destroyed by flood. They are not indebted. Need : Personal needs and children’s school needs must be provided.	Rs.4,000 US$.40.-
2	House: The house was made of clay walls, tile roof and clay floor. The house was fully damaged by the flood. They now live in a temporary shelter. She needs help to build a house. Need :She has been able to build her house with a grant of Rs.100,000/- from the National Housing Development Authority and a Belgian NGO that was building houses in the area. She needs to get an electricity connection to her house and dig a well for domestic water.	Well  Rs. 20,000 Electricity Rs.25,000 Rs.45,000 US$. 450.-
3	Household Goods: She had a almirah (cupboard), table, 6 chairs, cassette radio and some kitchen utensils; they were fully destroyed by the flood and need to be replaced. Need : She needs basic furniture, radio cassette and kitchen utensils.	Rs. 4,000 US$. 40.-
4	Vehicles: She did not have any vehicles. Need: Would like to have a push bicycle.
5	Farm Holding:  She had cultivated 4 acres upland in corn 2 ½ months before the floods. She has spent around Rs. 12,000/- for the cultivation. This field was completely destroyed by flood. She has to restart cultivation in October. Need: They need support to cultivate corn on their land.	Corn-Rs.25,100 US$.251.-
6	Other: She now earns about Rs.5,600/- and receives Rs.615/- from Samurdhi and Rs.250/- from another organisation; altogether Rs.6,630/-. Need : They need family support for 8 months @ Rs.3,000/-	Rs.24,000 US$. 240.-

8	Implementation Procedure: BDS has very kindly offered to undertake the field level implementation for the 20 families we are Adopting parallel to the projects it is implementing for other families in the area. Chakra Network Sri Lanka will take overall responsibility for the 20 families. This will involve receiving funds from Sponsoring Families / Individuals earmarked for the families they adopt, transferring funds to BDS for specific tasks in the Rehabilitation Plan for each Adopted Family, keeping track of funds spent for each Adopted Family and keeping the Sponsoring Families / Individuals informed of the progress in implementing the Rehabilitation Plan. This will incur some cost for Chakra Network Sri Lanka Need : Administration / Coordination cost	10,000
